In order to improve student skills proficiency and increase the student’s ability to think critically, the Germanna nursing program has integrated high fidelity simulation through the five semesters of the program. During the first semester of the nursing program, Nursing 110, teaching is focused on the ability of student’s formation of basic nursing skills. These skills include vital signs, physical assessments, recognizing deviations in patient baselines. High fidelity simulation experiences include introducing nursing students to vital sign collection and assessment. During this time students simulation experiences facilitate student’s ability to complete a physical assessment that includes assessing heart, lung, and bowel sounds. During Nursing 110 students spend two hours in the simulation lab.
Nursing 113 builds upon the previous semester skills and introduces the students to medical and surgical anatomy and physiology, long term care acute and chronic care issues, health promotion, patient rehabilitation, and well child care. Simulation experiences build upon the initial assessment skills and further the assessment with in-depth cardiac and respiratory evaluation. During the second semester Nursing 113 course, students spend 4 hours in the simulation lab.
Nursing 118 is in the third semester of the nursing program. During this time students are further introduced to medical surgical pathophysiology , geriatric, end of life issues, maternity and well baby care. Simulation experiences encompass obstetric assessment, well baby assessment, gastrointestinal pathophysiology with surgical interventions and the resulting assessments. During this course time, student simulation exposure is increased to 8 hours.

Nursing 208 begins in the fourth semester of the nursing program. This course progresses students to advanced medical surgical nursing while introducing mental health issues within the community. This semester also launches nursing students into leadership roles in nursing. Nursing 208 simulations expose students to simulations with multiple system failure and exposure to care of simulated pain in the baby. Simulation time for Nursing 208 is 4 hours.
Nursing 208 (second portion of course)) is concurrent in the fourth semester of the program and furthers the student’s knowledge base of professional issues of nursing. Simulation time during Nursing 208 is 2 hours and further builds on student’s physical assessment and evaluation with nursing interventions.
During the final semester of the nursing program, Nursing 214 course focuses on continuing the advanced medical surgical anatomy and physiology. This course also addresses leadership issues and managing community pediatric care. Simulation experiences include assessment for critical care, code situations, and a furtherance of the pain issue of caring for babies. Simulation time for the Nursing 214 course is 4 hours.

| Virtual Hospital Course Integration through the Curriculum |

Another use of high fidelity simulation at Germanna is the Virtual Hospital experience. Virtual Hospital is a simulated clinical experience that allows students to gain valuable clinical experiences in the simulation lab. The Germanna nursing program introduces nursing students to Virtual Hospital during the third semester. During the students Nursing 118 course, nursing students have an introduction to Virtual Hospital. Nursing 118 students are exposed to adult congestive heart failure, chronic obstructive pulmonary disease, pediatric bronchiolitis, and asthma scenarios. This introduction to Virtual Hospital is over six hours. During the fourth semester of the nursing program, Nursing 208 students participate in Virtual Hospital Basic. During this exposure, nursing students experience adult congestive heart failure, chronic obstructive pulmonary disease, diabetes mellitus, cerebral vascular accident, obstetrics, pediatric bronchiolitis and asthma scenarios. Virtual Hospital hours during Nursing 208 are six hours. Virtual Hospital Advanced is introduced in the fourth semester with the same scenarios as Virtual Hospital Basic with the addition of obstetric hemorrhage and intensive care hemorrhage. Virtual Hospital Advanced adds six hours to the total for semester four of the nursing program curriculum. During the final semester of the nursing program, Nursing 214 students partake in Virtual Hospital Complex. Virtual Hospital Complex incorporates the previous semester scenarios and expands the student’s roles as charge nurse/team leader.
According to Wilford and Doyle (2006), virtual clinical experiences are simulation scenarios “that allow the lecturer/facilitator to teach multiple learning objectives using simulation. The tool provides a framework that allows the teacher to set up and run the simulator, provide learners with pre-simulation reading, offer teaching questions, suggest minimal behaviors and enhance the evidence base” (p.496).
